- Birth Certificate or Affidavit
- Letter of Sponsorship/Scholarship
- Medical Fitness Certificate from Government Hospital
- Ten pieces of Passports Size Photograph
- One-Off ID Card Fee N8,000.00 (Valid for 4 years).
